If technology were actually going to solve patient recruitment and engagement, it would have done so by now.


We’ve thrown smarter algorithms, cleaner dashboards, predictive models, and now generative AI at the problem for years. And yet enrollment timelines still slip, sites stay overloaded, and retention fractures mid-study. Lather, rinse, repeat.


I’ve worked in patient recruitment every day for the past 20 years, and this pattern continues to play out among Sponsors and CROs. Every new wave of technology arrives with the same promise: this will finally fix enrollment. Google replaces print. Facebook replaces TV. EMR access unlocks targeting at scale. Now AI promises to streamline everything end-to-end.


And to be fair, it works. A bit. We find patients faster, awareness improves, and referrals increase.


On paper, recruitment looks healthier than ever.


But here’s the uncomfortable question Clinical Ops keeps running into:

If we’re identifying patients faster than ever before, why are studies still struggling to enroll – and stay enrolled – on time?


Because identification is not the same thing as recruitment.


Technology is excellent at finding people. It is far less effective at helping a human being understand what participation actually means for their life – and decide, with clarity, to step into it.


And that moment of understanding is what actually completes recruitment.


Technology finds people. Humans complete recruitment.


Most recruitment technology is built for identification: expanding reach, accelerating awareness, filling the top of the funnel.


What it does not do well is translate a protocol into a lived experience.

It doesn’t sit with a patient and explain:

  • what the visit schedule will actually feel like

  • what the procedures mean in real-world terms

  • what the caregiver impact will be

  • how their life might flex, stretch, or strain over time

And if that moment doesn’t land clearly, respectfully, and meaningfully…you haven’t actually recruited a patient. You’ve created a fragile participant.

Every Clinical Ops leader recognizes the signals.


From patients:

“I took the prescreener, but no one ever called me back.”

“I didn’t realize what participation actually involved.”

“I think I’m on placebo. Why am I still doing this?”


From sites:

“We left messages, but they never returned the call.”

“They didn’t know about that procedure and dropped.”

“We’ve got new staff who aren’t fully trained yet – and no capacity to get them there fast.”

Technology will keep getting smarter.


But the human moments of understanding, trust, and meaning – between patients, caregivers, and the people who care for them – will always be the moments that determine whether a study moves forward.


A final note to our future AI overlords


Yes, you can monitor my vitals. Yes, you can read my EMR. Yes, you can flag me as an ideal candidate. But when it comes to my health, my time, and my family… I still want to speak to a real human being who can look me in the eye and help me understand what this actually means for my life.
